What are the components of the registration process?Updated 3 months ago

The registration process includes the following:

  1. Health intake questions that enable the coach to better understand your health challenges and needs and to better support the coach in providing personalized feedback and recommendations.

  2. Sample collection timing questions and how-to videos

  3. Signature process for a series of agreements that include our privacy policy,  cancellation policy, telehealth informed consent, and disclosures. 

  4. The online completion of the intake form including the date/time of your sample collection. (it is not necessary to complete any written forms)

Each lab will contain an Orange insert card containing the QR code needed to register the lab. Please scan that QR code and also include the Orange insert card into the lab kit box when you return your sample.

Some kits may also include a Pink card or printed requisition form. If your kit has a Pink card or paper requisition form, please read below: 

For the following tests, an additional Kit ID is needed to register the lab. This Kit ID will be found on the Pink insert card or printed requisition form.  Also, be sure to return the Pink card or printed requisition form into your kit box when you return your lab sample.  (It is not necessary to actually fill in any of the printed requisition form; you only need to take the Kit ID and include it as part of the registration.)

Your registration is NOT complete without entering the date of collection. 



Please note: Your lab test kit cannot be tested until your registration is fully submitted and complete. If your sample is mailed back before you finish registering, your sample will be discarded within 5 days.

You will know that your registration is completed by seeing a “Registration Confirmation” page. You will also receive a confirmation email. Further, In your myhealth.equi.life account, you will see the button update to “View Registration”.
