What’s included in my lab test kit?Updated 10 days ago

Each lab test kit will include the following:

• Instructions - Please read the detailed printed instructions included with each lab test kit to understand how to register your lab and how to collect your specific lab sample(s)

• Testing components - Each test kit includes the components necessary to take  that specific test. 

• Orange insert card - The orange card includes the QR code needed to register your lab and also needs to be included in the kit box when you return your lab sample.

• Pink insert card - Some labs may also include a pink insert card or a printed requisition form. If your kit includes either a pink card or the requisition form, please include the Kit ID Number found on either of those documents, as part of your lab registration. Also, be sure to return the pink card or requisition form into your kit box when you return your lab sample.  (It is not necessary to actually fill in any of the printed requisition form; you only need to take the Kit ID and include it as part of the registration.)

• Return packaging - 

  • Domestic lab kits will include an envelope along with a  prepaid return label. Please follow the instructions included with your kit.
