How do I create an EquiLife Health Portal account?Updated 5 months ago

Most clients “Create Their Account” when either purchasing from EquiLife, when Registering their first lab test, or when they are invited by a Provider, but if you have not yet created an account, it only takes a few minutes to follow these easy steps:

1. Go to the Client EquiLife Health Portal at:  https://myhealth.equi.life/">https://myhealth.equi.life 

2. Click “Sign In with EquiLife”

3. On the next screen click “CREATE NEW ACCOUNT”

4. You will then be prompted to create and enter a username which should be the same email address if you have created any other EquiLife account
5. Then, click “Next”

6. Enter your first and last name

7.  Set and confirm a password and click “Create Account”

After you create your account, you must then visit and login to https://myhealth.equilife to gain access to the EquiLife Health Portal.

Very Important Notes:

• Every individual needs their own account with a unique email address. 

• If you are registering for children or family members, you must create a separate account with a unique email address for each individual.
