There is no need to apply. 

You will be automatically enrolled in the Ayubowan Membership Program upon your purchase of $79 or more of EquiLife qualifying subscriptions. 

All subscriptions qualify, regardless of frequency.  For example a 2-week, 4-week, 6-week subscription all count towards your subscription total.
